The below form has been handed out to your athlete and can be printed by clicking HERE!

Due to the success of the 2012-2018 Tax Credit Programs (TCP), we are again offering this TCP for the 2019 Football Season.  In order to help offset some of the personal expense that parents incur, and to improve the Sunnyslope Football Program, your donation will provide the following:

Raise $200 in Tax Credits: 

  • Sunnyslope Football will purchase a pair of cleats for your athlete from a select group of cleats, retail value approximately $90-$100 as listed on flyer.​

Raise $400 in Tax Credits:

  • Sunnyslope Football will purchase a pair of cleats for your athlete from a select group of cleats, retail value approximately $100 AND​

Sunnyslope Football will purchase one of the two pairs of gloves on the flyer AND the ALL IN ONE girdle worn under the uniform.  These items are yours to keep.

Contributions can now be made in two ways as follows:


  1. Check/Cash- Completed tax credit form (obtain on Sunnyslope High School web site) with cash or check, give to Coach Pieri-Varsity, Coach Tolman-JV or Coach Fenton-FR (ONLY), made out to Sunnyslope High School, 35 W. Dunlap, Phx, AZ 85021 and identify the activity/group as “Football”  AND the flyer with the items circled, sizes stated, and players name and phone number, OR

  3. Credit Card-Use your credit card by making an online payment through PayPal on our district’s website at http://www.guhsdaz.org, identify the school as Sunnyslope and the activity/group as “Football”.  Give receipt to the above coaches along with flyer with the items circled, sizes stated, and players name and phone number.



 We will do 2 orders this year, the first due Monday, May 20th to give Coach Pieri. Coach Tolman, or Coach Fenton the completed form/check or credit card receipt.  A second order has a deadline of August 12th , and we will make every effort to get the shoes/gear before the games start!  This deadline is not flexible, so get your order in early!
